My Living Will

Living Will for Advance Care Planning is a free service. The online registration consists of two parts; the Advance Statement of preferences and wishes (not legally binding; and the Advance Decision to refuse treatment (a legally binding document which must be respected).

Benefits:

  • Individual - can explore worries about end of life care; have peace of mind; plan for emergencies before they arise; and ensure no over-treatment.
  • Family - prevents conflict between siblings; saves the family anguish in a crisis; knows the family has done its best.
  • Professionals including carers, nurses, doctors and residential staff -  can have a better knowledge of the patient; develop constructive plans; be empowered to deliver better care. With all staff working towards the same goals there is less conflict.
