The Support at Home service is usually for older people and is designed to assist users with everyday activities following a stay in hospital or personal crisis in the community.
Puts kindness into action to support, care and help you live independently at home or when you return after a stay in hospital.
Provides up to four home visits over a four to six week period giving assistance with tasks such as shopping and prescription collection, completing forms, going for walks and providing an escort for appointments.
The Next Steps Re-enablement Service assists users to regain independence and re-learn skills which are essential for daily living.

developing a routine for carrying out domestic chores and starting a local hobby or interest.
The service does not provide transport or nursing / personal care and alcohol or drug users or those with aggressive behaviour
